Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver home-based SLP services and support rehabilitation goals. The therapist will evaluate patients, develop plans of care, and coordinate with families and caregivers to maximize independence and quality of life.

The role requires ASHA CCC, a license, CPR certification, and a valid driver’s license, with pay per visit ranges and annual base compensation described by Humana.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to support rehabilitation.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Document changes in patient condition and progress as required by policy.
• Provide instruction to patients in alternative communication methods when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain and submit documentation and progress reports.
• Participate in care coordination and discharge planning.
