Job summary

CenterWell Home Health in Joliet, IL is seeking a Speech Therapist to help patients regain strength and independence, delivering rehabilitation services in patients' homes or facilities.

You will evaluate, treat, and develop plans of care, coordinate with teams, document progress, and provide education to patients and families. Per diem and full-time opportunities exist across Will, Ford, and Kankakee counties.

Qualifications

• ASHA Clinical Competence certification or eligibility.
• Minimum six months experience as a speech-language pathologist.
• Home health experience is a plus.
• Current and unrestricted license.
• Current CPR certification.
• Good organizational and communication skills.
• A valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services at the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat speech, voice, language, hearing and swallowing disorders.
• Administer and interpret diagnostic tests including speech screenings.
• Observe and report changes in patient condition to supervisor or physician.
• Educate patients and families on communication strategies as appropriate.
• Document evaluations, visits, care plans and progress reports.
• Participate in care coordination and discharge planning.
