Job summary
CenterWell Home Health in Illinois is seeking a Speech-Language Pathologist (SLP-CCC) to deliver compassionate in-home therapy focused on rehabilitation and functional improvement. Qualifications
- Minimum six months experience as a speech-language pathologist.
- Current and unrestricted SLP license and CCC-SLP credentials.
- Current CPR certification is required.
- Valid driver's license, auto insurance and reliable transportation required.
Responsibilities
- Evaluate and provide speech-language pathology services in patients' homes or facilities.
- Participate in Plan of Treatment and Plan of Care development and review.
- Assess and treat disorders of speech, voice, language, hearing and swallowing.
- Administer and interpret diagnostic tests and therapeutic interventions.
- Document patient status and progress and report changes to supervisor or physician.
- Provide training to patients and families in communication strategies.
- Coordinate care and participate in discharge planning.
