Job summary

RVNAhealth is seeking a compassionate Registered Nurse Case Manager to provide skilled home health care across our Northern service area, including New Milford, CT. You will manage your own patient caseload, collaborate with an interdisciplinary team, and support patients in recovering, managing chronic conditions, and staying safely at home.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in Connecticut or Compact RN License.
• At least one year of nursing experience.
• Strong clinical assessment, critical thinking, communication, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Home health experience and OASIS documentation experience are a plus but not required.

Responsibilities

• Manage a caseload of home health patients with skilled nursing care and case management.
• Conduct Start of Care admissions, comprehensive assessments, and ongoing evaluations.
• Develop and update individualized plans of care with physicians and the care team.
• Coordinate nursing, home health aide, therapy, and other services for seamless care.
• Monitor progress, adjust care plans, and obtain physician orders as needed.
• Educate patients and caregivers on diagnoses, medications, and self-management.
