Job summary

Masonicare Home Health & Hospice in Norwich, CT seeks a full-time Homecare Scheduler for day shift (40 hrs/wk) to coordinate and maintain daily scheduling of home health aide visiting staff, ensuring patient needs are addressed and continuity of care.

Qualifications

• Education: High School Diploma or equivalent required; some college preferred.
• Experience: 2–3 years in scheduling of field staff.
• Experience with computerized scheduling software.
• Must possess reliable transportation and a valid Driver’s License

Responsibilities

• Input schedules into the computer scheduling system under the direction of the Supervisor.
• Schedules visits based on continuity of care for both staff and patients.
• Coordinates scheduling adjustments needed on a daily basis while monitoring and reporting assigned visits not completed.
• Communicates with patients regarding visiting staff schedules and schedule changes as needed.
• Builds associated personnel into patient files and reviews schedules to assure maximum consistency of visit staff for each patient scheduled.
• Assigns visits based on maintaining a low re-hospitalization rate.
• Matches schedules to ensure that all visits are completed in a timely manner by the staff.
• Performs functions to support the clinical verification process as needed
• Communicates effectively and tactfully with patients and/or their families, recognizing their age, cultural diversity, needs, abilities and physical condition.
• Performs other duties as assigned.
