Job summary

Enhabit Health is seeking a dedicated nurse scheduler to coordinate patient assignments and ensure seamless communication among field clinicians, patients, and physicians. You will manage on-call duties and hospitalization logs to support continuous, high-quality home care services.

The role requires an active LPN/LVN license, CPR certification, and at least one year of relevant healthcare experience, with strong Microsoft Office skills and reliable transportation.

Qualifications

• High School Diploma or equivalent required.
• Licensed Practical or Vocational Nurse (LPN/LVN) is required.
• Valid clinical license per state requirements.
• Minimum 1 year of clinical healthcare experience.
• CPR certification required.
• 1 year of home health, hospice, or pediatric experience in last 24 months preferred.
• Valid state driver license and automobile liability insurance.
• Proficiency with MS Office (Word & Excel) and mobile devices.
• In-office attendance is essential (except for designated full-time remote roles).
• Preferred bilingual English/Spanish communication.

Responsibilities

• Schedule patients to field clinicians.
• Coordinate with field staff, patients, physicians, referral sources and caregivers for care continuity.
• Manage on-call notebook and hospitalization logs to improve communication among stakeholders.
