Job summary

Enhabit Home Health & Hospice is hiring to coordinate patient scheduling with field clinicians and to support seamless care delivery. The role emphasizes strong communication with patients, staff, physicians, and caregivers to maintain continuity of care and timely responses.

Excellent organizational skills and HIPAA awareness are essential. Candidates should have an LPN license and at least one year in home health or hospice.

Qualifications

• High School Diploma or equivalent.
• Licensed Practical Nurse (LPN) license valid in state.
• CPR certification required.
• Minimum 1 year of clinical experience in healthcare.
• Minimum 1 year in home health, hospice, or pediatric care within last 24 months.
• Valid state driver license and automobile liability insurance.
• Basic technology skills and mobile device operation.
• Preferred experience with Homecare Homebase (HCHB).
• Preferred bilingual (English/Spanish) communication.
• Must have dependable transportation and safe driving in all weather.
• Strong oral and written communication with active listening.
• Intermediate MS Office skills (Word, Excel).

Responsibilities

• Schedule patients to field clinicians and ensure care continuity.
• Communicate with field staff, patients, physicians, and caregivers to coordinate care.
• Manage on-call notebook and hospitalization logs to support stakeholders.
