Humana, Inc. is seeking a Patient Services Coordinator for CenterWell Home Health in Kentucky. The role focuses on scheduling visits, coordinating with field staff, patients, physicians, and teams to maintain care continuity, and assisting with day-to-day office management.

• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsabilidades

• Schedules patient visits and coordinates with field staff, patients, physicians, and other teams to ensure smooth care coordination.
• Manages and edits schedules, reassigns patients due to staff unavailability, and updates agent availability in the worker console.
• Generates and reviews scheduling reports and prepares weekly staff schedules for Branch Director approval.
• Handles administrative tasks including infection control forms, lab report routing, and internal transfers as needed.
