Job summary

CenterWell Home Health is seeking a Patient Services Coordinator to manage patient scheduling, liaise with field staff and physicians, and support day-to-day office operations. The role emphasizes strong communication, organization, and coordination across multiple stakeholders to maintain care continuity.

The position requires at least 1 year of home health experience, with preferred familiarity in QA and reporting.

Qualifications

• Excellent communication and interpersonal skills with diverse groups.
• Strong organizational abilities and prioritization of multiple tasks.
• At least 1 year of home health experience.
• Coding certification is preferred or not required; QI experience is a plus.

Responsibilities

• Schedule management for all patients and reassignments when staff are unavailable.
• Process infection control forms and coordinate with HRD/medical records as needed.
• Support field staff with calls, case conferences, and weekly scheduling tasks.
• Prepare and review scheduling reports and weekly agent schedules for productivity and geographic issues.
• Assist with patient transfers between branches and manage lab reports routing when clinical.
