CenterWell Home Health is seeking a Patient Services Coordinator to manage patient visit schedules and coordinate with field staff, patients, and physicians to maintain continuity of care. The role also supports day-to-day office and staff management.

Responsibilities include managing schedules, processing infection control forms, assisting with case conferences, and ensuring supervisory and add-on visits are properly arranged. May participate in on-call rotations as needed.

Formación

• Must have excellent communication skills and the ability to interact with a diverse group of individuals.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsabilidades

• Manages schedules for all patients and updates agent availability as needed.
• Initiates infection control forms and routes documents to the appropriate department.
• Supports busy periods, field staff calls, and case conferences.
• Keeps hospitalisation log up to date and shares coordination notes.
• Ensures appropriate scheduling of skilled nurse or injection visits and add-ons.
• Schedules discharge visits and OASIS collections, and parses scheduling tasks as they appear.
• Reschedules declined or missed visits and handles reassignment of visits.
• Verifies visit notes in scheduling console and assists with intra-branch transfers.
• Participates in on-call rotations if required by clinical duties.
