Job summary

CenterWell Home Health in Danville, KY is seeking a Patient Services Coordinator to schedule visits and coordinate care with field staff, patients, physicians, and others to ensure continuity of care. The role also supports day-to-day office operations and staff management.

Qualifications

• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Schedule visits and manage patient schedules; reassign when staff are sick.
• Initiate infection control forms and file infection reports.
• Support staff during busy times and assist with weekly case conferences.
• Maintain hospitalization log and coordinate lab routing as needed.
• Review and prepare weekly schedules for staff and track productivity.
• Process reassigned and rescheduled visits and ensure supervisory visits.
• Assist with internal transfer of patients between branches.
• May perform patient visits or participate in on-call rotation if clinical.
