Humana, Inc.’s CenterWell Home Health is seeking a Patient Services Coordinator to manage scheduling of visits and coordinate with field staff, patients, and physicians to maintain seamless care. The role supports daily office operations and ensures continuity of care across the network.

You will handle staffing adjustments, run scheduling reports, assist with infection control and HRD processes, and may participate in on-call rotations.

Formación

• Must possess excellent communication skills and interact well with a diverse group.
• Strong organizational skills and the ability to manage and prioritize multiple assignments.
• Minimum 1 year of home health experience.
• Packet review / QI experience preferred.
• Coding certification is preferred.

Responsabilidades

• Schedule visits for patients and coordinate with field staff, physicians, and other care providers.
• Edit schedules and reassign patients promptly when staff are sick or unavailable.
• Maintain scheduling logs and generate reports for staffing and productivity.
• Assist with infection-control forms and HRD processes as needed.
• Provide back-up support during peak times and participate in on-call rotation.
