Job summary

CenterWell Home Health is seeking a Patient Services Coordinator to schedule visits and coordinate with field staff, patients, and physicians to maintain care continuity. The role also supports day-to-day office and staff management.

The position requires strong communication, organization, and multi-tasking abilities, with at least 1 year of home health experience. Codin g certification is preferred and QI experience is a plus.

Qualifications

• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Manage schedules for all patients and reassign as needed.
• Coordinate with field staff, patients, physicians to maintain care continuity.
• Initiate infection control forms and route reports to HRD and Medical Records as needed.
• Support office operations and staff management.
• Prepare weekly scheduling reports and supervise staff scheduling.
• Assist with on-call coverage and case conferences.
• Assist with transfers of patients between branch offices.
• Review lab reports and route to Medical Records.
