Job summary

CenterWell Home Health in Charlotte, NC is seeking an experienced Scheduling Coordinator to manage patient visits and adapt to staff changes. Qualifications

• Must possess excellent communication skills, the ability to interact with a diverse group of individuals, strong organizational skills and the ability to manage multiple assignments.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Manages schedules for all patients and edits schedules for coverage when agents are sick.
• Initiates infection control forms and coordinates with HRD for uploading in the worker console.
• Backs up during busy times and handles calls from field staff; participates in weekly case conferences.
• Maintains the client hospitalization log and coordinates notes with office, field, and sales staff.
• Completes requested schedules for add-ons and applicable orders; schedules discharge and OASIS collection visits.
• Reschedules declined or missed visits and processes reassigned and reassigned visits.
• Ensures supervisory visits are scheduled and runs scheduling reports such as Agent Summary and Missed Visits Done on Paper.
• Verifies visit paper notes in scheduling console and assists with transferring patients between branches.
