Job summary

Four Seasons Healthcare Solutions is seeking a Scheduling Coordinator to manage CHHA staff schedules and assignments, ensuring timely home care for patients. You’ll coordinate visits, handle conflicts, and communicate with patients, families, and the care team.

The ideal candidate will have strong communication skills, familiarity with home care software, and at least one year of scheduling experience in a fast-paced environment.

Qualifications

• Minimum 1 year Scheduling Coordinator experience.
• Familiarity with home health regulations and policies.
• Experience with high-volume coordination preferred.

Responsibilities

• Plan and coordinate CHHA visits to ensure adequate coverage and timely care.
• Communicate schedule changes with patients, families, caregivers, and team members.
• Maintain accurate records of visits and scheduling adjustments in agency systems.
• Collaborate with nurses, case managers, and other staff to ensure coordinated care.
• Provide excellent customer service to patients and families.
