Qualifications

• High school diploma or equivalent preferred.
• Experience in home health scheduling is preferred.
• Ability to multitask in a fast-paced environment.
• Strong communication and interpersonal skills.
• Familiarity with Netsmart, HHA Exchange, and other scheduling systems.
• Knowledge of home care regulations and policies.

Responsibilities

• Plan and coordinate CHHA visits with adequate coverage.
• Address scheduling conflicts and resolve staffing issues.
• Communicate with patients, families, and caregivers about schedules.
• Maintain accurate records of visits and scheduling changes.
• Ensure assessments and visits are scheduled (30 days rehab reassessment, recert, ROC/SOC).
• Maintain productivity logs and report progress.
• Collaborate with nurses and case managers for coordinated care.
• Answer calls and manage messages.
• Provide information to clients, providers, and staff.
