Job summary

Enhabit Home Health & Hospice is hiring a nurse scheduler in Longview, TX to coordinate patient visits with field clinicians. You will manage scheduling, communicate with patients, physicians, and care teams, and support care continuity across home-based services.

The ideal candidate is an LPN/LVN with a valid license, CPR, and at least one year of clinical experience, including home health or hospice. On-site work is required, with strong communication and tech skills.

Qualifications

• High School Diploma or equivalent.
• Licensed Practical or Vocational Nurse with valid state license.
• CPR certification required.
• At least 1 year of clinical healthcare experience.
• 1 year home health, hospice, or pediatric experience in last 24 months.
• Valid state driver's license and automobile liability insurance.
• Basic technology skills and operation of a mobile device.
• Preferred: Homecare Homebase (HCHB) experience.
• Preferred bilingual English/Spanish communication.
• On-site attendance required for full-time position.

Responsibilities

• Scheduling patients to field clinicians and coordinating care.
• Communicate with field staff, patients, physicians, and providers to ensure continuity of care.
• Manage on-call notebook and hospitalization logs to improve stakeholder communication.
