Job summary

Enhabit Home Health & Hospice in Lakeland, FL seeks a scheduling coordinator to support field clinicians and care teams. You will organize patient visits, coordinate with physicians, referral sources, and caregivers, and help maintain accurate care plans.

In this role you will manage the on-call notebook and hospitalization logs to enhance communication among stakeholders, ensure timely responses, and uphold Enhabit's commitment to compassionate, high-quality home care.

Qualifications

• High School Diploma or equivalent required.
• Licensed Practical Nurse (LPN) with active license.
• Valid CPR certification required.
• Minimum one year of clinical healthcare experience.
• Valid state driver license and automobile liability insurance.
• Strong technology skills including basic computer and mobile device operation.
• Experience with Homecare Homebase (HCHB) preferred.
• Bilingual English/Spanish is a plus.
• In-office attendance essential for supervision and teamwork.

Responsibilities

• Schedule patients to field clinicians and coordinate with care teams.
• Communicate with field staff, patients, physicians, referral sources, and caregivers to maintain care coordination.
