Job summary

Enhabit, Inc. is seeking a patient scheduler to coordinate scheduling for field clinicians and maintain seamless communication among patients, staff, and providers. The role emphasizes strong coordination, licensing compliance, and reliable transportation in the home health environment.

Ideal candidates have nursing credentials (LPN/LVN), CPR, and at least one year of healthcare experience, with preference for Homecare Homebase (HCHB) experience and bilingual English/Spanish skills.

Qualifications

• High School Diploma or equivalent.
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
• Valid clinical license per state requirements.
• Minimum of one year of clinical experience in a healthcare setting.
• Current CPR certification.
• One year home health, hospice, or pediatric experience in last 24 months.
• Valid state driver license and automobile liability insurance.
• Basic technology skills and mobile device operation.
• Preferred: Homecare Homebase (HCHB) experience.
• Preferred: bilingual English/Spanish.
• In-office attendance is essential (except for designated full-time remote positions).

Responsibilities

• Scheduling patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care coordination and continuity of care.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
