Job summary

Croasdaile Village in Durham, NC is seeking a Home Care RN for a PRN Day Shift with weekend availability and occasional weekdays. The RN will plan, organize and direct home care services, building on community resources to meet patient and family needs in their homes and communities.

Qualification: Graduate of an accredited nursing program; NC RN licensure and CPR; valid driver’s license; 1–2 years home health care preferred; on-call duties; excellent communication.

Qualifications

• RN licensed in North Carolina with CPR certification.
• Experience in home health care preferred.
• Excellent communication and patient education skills.

Responsibilities

• Plans, organizes and directs home care services for assigned patients.
• Completes initial assessments and develops care plans with patient and family.
• Coordinates care with physicians and community resources.
• Provides direct patient care and administers prescribed treatments.
• Assists with discharge planning and on-call duties as needed.
• Educates patients and families on health conditions and self-care.
