Job summary

3HC Inc. is seeking a Home Health and Hospice RN to join our team in Pollocksville, NC. This full-time role involves on-call duties from 5pm Friday through 8am Monday, delivering skilled nursing care to Home Health and Hospice patients.

The RN will develop, implement, and monitor patient care plans, coordinate with clinicians, educate families, and supervise nursing staff. Strong communication and clinical skills are essential.

Qualifications

• Registered Nurse licensed to practice in North Carolina; minimum of one year experience as a professional nurse in an acute care setting.
• Maintain current CPR certification (BCLS).

Responsibilities

• Admit patients to the Agency with medical history and physical assessment.
• Develop and implement the patient care plan with other Agency staff.
• Manage assigned cases and provide skilled nursing care per physician plan.
• Educate and counsel patients and families on nursing care needs and end-of-life care.
• Supervise Nursing Assistant, ensure plan of care is followed and updated.
