Job summary

Croasdaile Village in Durham, NC is seeking a Home Care RN for PRN daytime shifts with weekend availability. The RN plans, organizes and directs home care services, delivering nursing care in patients' homes with emphasis on community health and education.

Current RN licensure and CPR are required, with 1–2 years of home health experience preferred. We offer a comprehensive benefit package and equal opportunity employment.

Qualifications

• Graduate of an accredited nursing program with current licensure in the state.
• CPR certification required.
• 1–2 years of healthcare experience preferred.
• Experience in home health care is preferred.
• Ability to conduct comprehensive patient assessments and care planning.
• Prolonged walking/standing; ability to lift and transfer patients.

Responsibilities

• Completes initial patient and family assessments to determine home care needs.
• Regularly reevaluates patient nursing needs.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to determine nursing diagnoses.
• Develops care plans with goals based on nursing diagnosis.
• Administers medications and treatments as prescribed; provides direct patient care.
• Counsels patients and families on nursing needs.
• Provides health care instructions per plan of care.
• Identifies discharge planning needs and coordinates care.
• Acts as Case Manager when assigned.
