Job summary

Duke HomeCare & Hospice provides hospice, home health, and infusion services across the region. This role focuses on delivering high-quality nursing care in patients' homes or at the inpatient hospice facility in Durham, NC, coordinating with physicians and interdisciplinary teams to achieve optimal outcomes.

Expect to assess patients, develop care plans, educate families, and document care in compliance with NC regulations and agency standards.

Qualifications

• Registered nurse (RN) with NC license or compact state
• CPR certification (AHA)
• Strong assessment, critical thinking and documentation skills
• Excellent communication and ability to work independently in home settings

Responsibilities

• Deliver and document professional nursing care in home settings using nursing process
• Conduct comprehensive assessments and develop individualized care plans with the care team
• Educate patients and families on disease processes, medications and care plans
• Coordinate care including case management, discharge planning and OASIS-related activities
• Ensure regulatory, safety and quality standards and timely documentation
