Duke Health's Duke HomeCare & Hospice seeks a Care Coordinator at Duke Raleigh Hospital to coordinate home health, hospice, and infusion services for inpatients needing home care. You will assess needs, educate patients/caregivers, and support safe home management and discharge planning in collaboration with the care team.

The role requires a BSN and NC RN licensure, BLS, and 3 years of nursing experience. Work primarily Monday–Friday, 8:30 AM–5:00 PM with on-call rotation every 4–6 weeks.

Formación

• BSN required and active RN licensure in NC or compact state.
• BLS certification required (renewable during orientation).
• Three years of nursing experience required.

Responsabilidades

• Coordinate home health, hospice, and infusion services for patients at Duke Raleigh Hospital.
• Assess inpatient home care needs and educate patients and caregivers.
• Ensure safe home care management and discharge planning with interdisciplinary teams.
