Job summary

Duke HomeCare & Hospice provides hospice, home health, and infusion services across the region, delivering compassionate care in patients' homes or at our inpatient facility in Durham, NC. As a nurse with this team, you’ll work closely with physicians and the care team to support patients in vulnerable moments and support families with education and discharge planning.

Responsibilities include delivering professional nursing care, documenting care, and collaborating with the interdisciplinary

Qualifications

• Associate Degree or Diploma, Nursing required.
• BLS Certification is preferred and may be recertified during orientation.
• Current North Carolina RN Licensure or compact licensure in participating state.

Responsibilities

• Plan, provide, supervise and document professional nursing care using the nursing process in home or alternate-site settings.
• Collaborate with healthcare team to evaluate and ensure cost-effective patient care.
• Assess educational needs of patients and families and plan appropriate education interventions.
• Develop and implement discharge planning for patients starting at initiation of services.
• Maintain up-to-date knowledge of Medicare/Medicaid/JCAHO standards through inservices and workshops.
