Job summary

Duke HomeCare and Hospice provides hospice, home health, and infusion services across central NC. Nurses work in patients' homes or inpatient hospice facilities, delivering compassionate care and coordinating with interdisciplinary teams to achieve optimal outcomes.

Location-based roles in Durham, NC require an active NC RN license, CPR certification, and 1–2 years of relevant experience. Duke offers relocation support and a commitment bonus as part of its Total Rewards.

Qualifications

• RN license in North Carolina or compact state.
• CPR certification (American Heart Association).
• 1–2 years of med-surgical, critical care, or home health experience preferred.

Responsibilities

• Deliver and document professional nursing care in home settings using the nursing process in accordance with North Carolina regulations and agency standards.
• Conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to deliver effective care.
• Educate patients and families on disease processes, medications, and care plans to support outcomes and reduce hospital readmissions.
• Coordinate care including case management responsibilities, discharge planning, and OASIS assessments tied to reimbursement and quality outcomes.
• Ensure compliance with regulatory, safety, and quality standards while maintaining accurate and timely documentation.
