Qualifications

• Current Texas RN license is required.
• 1+ year of nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation and valid driver's license.
• Comfortable using EMR/mobile charting.

Responsibilities

• Perform in-home assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, and med administration.
• Educate patients and families on care plans.
• Coordinate with physicians and care teams; document thoroughly.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
