Job summary

Alliance for Care at Home is seeking a registered nurse to manage an assigned patient caseload in the home health setting. You will ensure timely care and documentation aligned with the plan of care and physician orders, and coordinate data collection from assessments and EMR, including telephonic contacts.

You will lead a multidisciplinary team, supervise LPNs and HHAs, perform treatments as required, and evaluate progress toward patient-centered goals while addressing barriers and maintaining

Qualifications

• RN license in the state where you work; BSN preferred; BLS required by policy.
• Experience in home health or similar settings is preferred.

Responsibilities

• Manage daily patient caseload and ensure timely care and documentation as prescribed.
• Collect data from assessments, home health visits, and EMR info.
• Identify high-risk patients and coordinate interventions to reduce rehospitalization or resource use.
• Prioritize critical situations to ensure patient safety and timely services.
• Identify barriers to resolution and coordinate with resources or leadership.
