Job summary

Advocate Health is seeking a skilled Registered Nurse to manage a patient caseload in the home setting. You will collect data from assessments, MD orders, and EMR to coordinate care with a multidisciplinary team and ensure high-quality outcomes.

Responsibilities include implementing plans, supervising LPNs/HHAs, performing treatments, and evaluating progress toward patient-centered goals while adhering to state regulations and policy guidelines.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• BLS required per policy guidelines.
• Current RN license or temporary RN license in the state where you work.
• If using the National License Compact, licensure in your home state.
• For non-NLC states, current RN license or temporary license as a Nurse Practitioner required in the state where the RN works.

Responsibilities

• Manage daily patient caseload and document per plan of care and MD orders.
• Collect data from assessments in home setting, telephonic contacts, and EMR.
• Analyze data to identify issues and resources; identify high‑risk patients.
• Prioritize multi‑issue situations to protect patient safety and ensure timely services.
• Coordinate care with a multidisciplinary team and supervise LPNs/HHAs involved in the care.
• Perform ordered treatments, therapies, and procedures per policy and regulations.
• Evaluate progress toward patient-centered goals.

Education

Bachelor of Science in Nursing

BSN preferred
