Qualifications

• Associates degree in nursing from an accredited program.
• Current RN license in the state of practice; BLS certification within 6 months.
• Experience in clinical nursing is typically required (3 years).

Responsibilities

• Conducts initial patient assessments and builds care plans.
• Provides direct patient care and oversees nursing activities.
• Completes documentation including OASIS and 485 promptly.
• Coordinates discharge planning and continuity of care.
• Communicates with physicians and healthcare team to update plan.
• Participates in quality improvement activities.
• Maintains patient records per agency/state guidelines.
• Delivers age-appropriate care across the lifespan.
