Job summary

Team Select Home Care is seeking an experienced registered nurse for home health care. You will develop and implement patient-centered care plans under physician supervision, provide case management, and handle essential documentation like OASIS and PAR within required timeframes.

You will coordinate care with families, physicians, and outside providers while administering medications and ensuring patient safety.

Qualifications

• Proficient in clinical documentation and data collection sets (OASIS, PAR)
• Strong organizational and supervisory abilities per state requirements
• Excellent verbal and written communication skills

Responsibilities

• Develops and revises patient-centered care plans under physician supervision
• Provides case management for an assigned group of patients and staff
• Submit required documentation (OASIS, PAR) within mandated timeframes
• Perform initial evaluations and periodic reassessments of nursing needs
• Advise patients and families, coordinating care with providers and facilities
• Administer medications per policy and orders and educate on home health strategies
• Document clinical notes and participate in case conferences as assigned
• Ensure confidentiality and maintain compliance with agency policies
