Job summary

Qualifications

• Graduate from an accredited School of Nursing.
• BLS required per policy guidelines.
• Current RN license in the state of work or residence; National License Compact state requirements may apply.

Responsibilities

• Manage daily patient caseload and ensure care per plan of care and MD orders.
• Collect data from assessments and EMR to inform care decisions.
• Identify high-risk patients to prevent readmission and coordinate with the team.
• Prioritize critical situations and safety to ensure timely services.
• Supervise LPNs and HHAs involved in patient care.
• Perform ordered treatments and evaluations toward patient goals.
