Team Select Home Care is seeking a qualified registered nurse to manage care for an assigned group of patients in a home health setting. You will develop, implement, and revise patient-centered care plans, coordinate with physicians, therapists and family members, and supervise ancillary staff as needed.

The role focuses on medication reconciliation, accurate documentation, and ensuring services meet the patient’s goals while adhering to state regulations and payer requirements.

Qualificações

• Must be a graduate of an approved school of professional nursing and currently licensed in the state(s) of practice.
• Minimum 1 year nursing experience; home health experience preferred.
• Ability to supervise ancillary staff and coordinate patient care.

Responsabilidades

• Develops and evaluates individualized patient-centered care plans.
• Provides case management for patients and families, coordinating care with providers.
• Maintains documentation, medication reconciliation, and progress notes.
• Participates in on-call duties and communicates changes to the Nursing Supervisor.
• Ensures compliance with state and payor requirements and agency policies.
