Job summary

Alternate Solutions Health Network seeks a dedicated Registered Nurse Case Manager to lead home health patient care from assessment through plan of care implementation. You will coordinate multidisciplinary teams, perform required assessments, update physician orders, and educate patients and families within regulatory guidelines.

Ideal candidates have RN licensure in the state, at least one year of acute care RN experience, and a commitment to high-quality, cost-effective care.

Qualifications

• Minimum of one year of RN experience in an acute care setting.
• Home care experience is desired.
• Valid RN license in the state of employment.
• Strong communication and patient-family education skills.

Responsibilities

• Oversees the overall care of the patient from start to end of episode.
• Updates doctor orders in patient chart and coordinates multidisciplinary care.
• Performs OASIS assessments to develop an individualized plan of care.
• Administers medications as prescribed and ensures safety.
• Provides education to patients and families and documents all care activities.
• Participates in on-call requirements and case conferences.
