• Assess and identify patient/family needs.
• Coordinate the Plan of Care as part of the IDT.
• Conduct home visits for assessment of safety and compliance.

Skills

Current license as RN

Minimum of one year nursing experience

Experience with EMR/EHR systems

Effective communication in English

Ability to perform physical functions of the job

Education

Active BLS certification
