Job summary

Compensation range is $69,720.00 to $103,771.25.

Qualifications

• Current RN license in the applicable state.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.
• Valid driver's license and automobile insurance.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patients and families about disease, palliative care, safety and caregiving.
