Qualifications

Responsibilities

• Manage direct care to patients and families.
• Coordinate individualized Plan of Care.
• Educate patients and families about care processes.
• Home visits to assess safety and compliance.
• Communicate changes in patient condition to IDT.

Skills

Current license as RN

Minimum of one year nursing experience

Experience with EMR/EHR systems

Effective communication in English

Ability to visit participants at home

Active BLS certification

Education

Current RN license in Maryland
