Enhabit Home Health & Hospice is seeking a Care Transitions Coordinator (Clinical Liaison) to help patients navigate post-acute care and to improve outcomes while enhancing referral source satisfaction.

You will integrate evidence-based guidelines into transition plans, and represent area branches in strategic relationships with health systems, hospitals, physicians and executive-level partners to advance quality and efficiency in home-based care.

Qualifikationen

• Must be a graduate of an approved school of nursing or therapy.
• Must be licensed in the state where they currently practice.
• Must have two (2) years of demonstrated field experience.
• Must have demonstrated understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Aufgaben

• Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care.
• Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities
