Job summary

Enhabit Home Health & Hospice seeks a qualified professional to support transitional care activities, coordinating with patients, families, physicians, and facilities to promote smooth care transitions.

You will develop evidence-based, patient-centered transition plans, ensuring quality and efficiency in care delivery while serving as a public awareness representative for hospice services and payor sources.

Qualifications

• Must be a graduate of an approved school of nursing, therapy, or social work.
• Must be licensed in the state where they currently practice.
• Must have at least two years clinical experience.
• Must have demonstrated knowledge of federal, state, and local laws and guidelines governing home health or hospice operations.
• Must have basic technology skills, including use of a mobile device.

Responsibilities

• Represent the branch in transitional care activities with patients, families, physicians, hospitals and facilities.
• Develop patient-centered transition plans that promote quality and efficiency in care delivery.
• Serve as a public awareness representative and manage education related to hospice services and payor sources.
