Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to support patients through post-acute care, driving better outcomes and higher referral satisfaction. You will apply evidence-based guidelines, coordinate transitions, and collaborate with health systems and physicians to deliver compassionate home-based care.

Ideal candidates are licensed nurses, therapists or social workers with field experience and strong regulatory knowledge.

Qualifications

• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Assist patients in navigating post-acute care to improve outcomes.
• Develop transition plans using evidence-based guidelines and protocols.
• Represent area branches in strategic relationships with health systems, hospitals, physicians and executives.
