Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to manage relationships with physicians, hospitals and referral sources. The role focuses on increasing referrals, educating partners on services, and guiding post-acute care transitions to improve patient outcomes.

Ideal candidates have a healthcare business background, strong interpersonal skills, and experience with regulatory guidelines. In-office presence is essential to support collaboration and care excellence.

Qualifications

• Bachelor's Degree in healthcare administration, marketing, or related field, or graduate of nursing, therapy, or social work.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Working knowledge of federal, state, and local laws governing Medicare certified home health and hospice.
• Valid state driver license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources, including physicians, hospitals, and health systems.
• Support referral growth by educating stakeholders about Enhabit services and program requirements.
• Help navigate post-acute care transitions to improve patient outcomes and satisfaction.
• Collaborate with clinical and operations teams to ensure high-quality patient care and service delivery.
