Job summary

Residential Home Health and Residential Hospice is seeking a Home Health Consultant in Lancaster, PA to partner with field staff and leaders. The role focuses on maintaining referral-source relationships, prospecting new sources, and coordinating care to ensure high-quality patient service across home health, journey, palliative, and hospice offerings.

Qualified candidates will have an Associate's degree (Bachelor's preferred), healthcare marketing experience, and a valid PA driver's license,

Qualifications

• Associates degree required; bachelor preferred.
• One year of healthcare marketing experience preferred.
• Understands federal and state marketing laws pertaining to home health.
• Must have valid PA driver's license.

Responsibilities

• Maintain relationships with all existing referral sources.
• Actively prospect for new referral sources based on the Agency’s scope of service.
• Coordinate care for patients by communicating with referral sources and internal clinical operations.
• Educate referral sources on Home Health and Hospice offerings.
• Provide timely follow up on patient statuses with referral sources.
