Job summary

Residential Home Health and Hospice is seeking a Home Health Consultant to partner with our field staff and leaders to support patients and referral sources, ensuring timely, high-quality care.

You will educate hospitals, skilled nursing facilities, assisted living facilities, independent living facilities, and physicians about Home Health, Journey, Palliative and Hospice offerings, while maintaining relationships and coordinating care.

Qualifications

• Associates Degree or higher is required or preferred.
• Experience in healthcare marketing is preferred.
• Knowledge of federal and state marketing laws is required.
• Commitment to high-level customer service for referral sources.
• Valid PA Driver's license is required.

Responsibilities

• Maintain relationships with existing referral sources.
• Actively prospect for new referral sources based on the Agency's scope of service.
• Collaborate with Marketing, Agency Administrator and Clinical Management to align demand with capacity.
• Educate referral sources on Home Health, Journey, Palliative and Hospice offerings.
• Provide timely follow up on patient statuses with referral sources.
• Coordinate care by communicating with referral sources and clinical operations.
• Assist in collection of orders and documentation for face-to-face encounters.

What you'll do in this role:

• Maintain relationships with all existing referral sources.
• Actively prospect for new referral sources based on the Agency's scope of service.
• Work closely with Marketing, Agency Administrator and Clinical Management to ensure that the demand for clinical care is aligned with our capacity and at no time compromises the Agency's ability to provide the best quality care for our patients.
• Educate referral sources on the Home Health, Journey, Palliative, and Hospice offering. This may include: hospitals, skilled nursing facilities, assisted living facilities, independent living facilities, and physicians.
• Provide referral sources with timely follow up on patient statuses.
• Coordinate care for patients by clearly and effectively communicating with referral sources and the internal clinical operations team.
• Aid in the collection of orders and face to face encounter documentation when necessary.

We are looking for compassionate Home Health Consultant with:

• Associates Degree, Bachelor Degree preferred
• One-year of healthcare marketing experience preferred
• Understands all federal and state laws pertaining to the marketing of home health and ensures total compliance with marketing staff.
• Committed to providing highest-level customer service to our referral sources.
• Must have valid PA Driver's license.
