Job summary
Constellation Health Services in Lancaster, PA is seeking a Part-Time RN Clinical Liaison to expand hospice and home health services. The RN will assess patients, develop care plans, and coordinate with hospitals, skilled nursing facilities, physician offices and community partners to meet home care needs.
The ideal candidate has 1–2 years of acute care experience, an active nursing license and CPR, plus strong communication and sales skills to generate Medicare referrals and build lasting
Qualifications
- Graduate of an accredited school of nursing and state licensure.
- 1–2 years of recent acute care experience in an institutional setting.
- Home health care experience preferred.
- Excellent communication and problem-solving skills.
- Ability to lift, transfer patients and lift supplies; valid driver's license.
Responsibilities
- Assess patient and family to determine home care needs and plan care.
- Develop care plan with goals based on nursing diagnosis; adjust as needed.
- Administer medications and treatments as prescribed by physician.
- Coordinate patient care as Case Manager when assigned by Clinical Supervisor.
- Generate Medicare referrals for hospice/home care by building relationships with social workers and physicians.
