Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to lead referral development and relationship management with physicians, hospitals, and health systems. You will educate partners on services and program requirements, supporting seamless post-acute transitions to improve patient outcomes.

The role emphasizes in-office collaboration, regulatory understanding, and driving growth in a dynamic home-based care environment.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or graduate of approved nursing/therapy/social work school.
• Strong interpersonal, communication and presentation skills.
• Knowledge of federal, state, and local laws governing Medicare home health and hospice operations.
• Valid driver's license and automobile liability insurance as required by law.

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, senior living communities, professional associations).
• Support referral growth by educating partners on Enhabit services and program requirements.
• Assist in navigating post-acute care transitions to improve patient outcomes and satisfaction.
