Job summary

Optum, in partnership with Amedisys, is seeking a Physical Therapist Assistant to provide skilled PT in patients' homes under a licensed PT’s supervision. You will implement individualized treatment plans to improve mobility and independence, while coordinating with patients, families, and the care team.

Requirements include CAPTE-accredited PTA education, current PTA license, CPR, and 1+ years of home-health experience or approval by regional clinical leaders for mentorship.

Qualifications

• Graduate from an accredited PTA program per CAPTE.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ years of PTA experience in home health preferred; if <1 year, mentoring/residency required.
• Driver's license and reliable transportation.

Responsibilities

• Provide physical therapy services under supervision of a licensed PT in patients' homes.
• Observe and report patient responses to treatment to the supervising PT and clinical staff.
• Document supervisory activities and patient progress in medical records.
• Coordinate with patients, caregivers, and care team for follow-up and referrals.
