Job summary

Amedisys, part of Optum, is seeking a Physical Therapist Assistant to provide skilled services in patients’ homes under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, balance and independence while promoting safety.

You will collaborate with patients, family caregivers and the care team to support recovery and enhance quality of life at home, with a comprehensive benefits package and opportunities for growth.

Qualifications

• Graduate from an accredited physical therapy assistant educational program as set by CAPTE.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ years of related PTA experience in home health care or, if <1 year, mentoring/residency program approval.

Responsibilities

• Provides physical therapy services and interventions under the supervision of a licensed physical therapist and in accordance with the physician approved plan of care.
• Observes, records and reports the patient's response to treatment and any change in condition to supervising PT, physician and staff.
• Provides resources for future needs and promotes wellness and prevention.
• Includes documentation of supervisory activities in the patient's medical record.
• Monitors and documents the patient's response to PT intervention.
• Participates in patient care conferences, in services, chart audits and quality improvement activities.
• Maintains confidentiality of patient information.
• Makes follow-up care recommendations coordinating with the admitting agency and community providers.
• Performs other duties as assigned.
