Commonwealth Home Health, part of the LHC Group and the Optum family, is seeking a Physical Therapist Assistant to perform treatment and provide exercise instruction and patient education under the plan of care.

You will follow state laws governing home PT services, assist with active and passive exercises, observe patient responses, and coordinate care with the healthcare team, including the supervising PT. Weekend rotation availability is required.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Available to work a weekend rotation

Responsabilités

• Follow state laws and the plan of care under the supervising PT; perform only procedures you are trained to perform and suggest changes to the plan of care as directed
• Instruct and assist patients in active and passive exercises, gait training, ADL, transfers, safety, and prosthetic training
• Observe, record, and report patient responses and changes to the supervising PT/nurse/physician; coordinate care with the healthcare team
• Educate patient, family, caregiver, and other health team members in therapy within the scope of the PTA
• Provide care in home settings regardless of age, race, creed, color, sex, disability, or financial condition
