VitalCaring Group in Kingwood, TX, is seeking a Physical Therapy Assistant (PTA) for Home Health to deliver hands-on therapy in patients' homes. You will work under a licensed Physical Therapist, guiding exercises, gait training, and mobility to improve function and independence.

This field-based role emphasizes one-on-one care, patient education, and coordinating with nurses and other clinicians. Candidates should have PTA graduation, current license, CPR, and a clean driving record.

Formación

• Graduate of an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as required by state law.
• 1+ year of experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsabilidades

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Utilize modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
• Observe and report patient progress and changes in condition.
• Maintain accurate documentation in the EMR and coordinate with the care team.
