Adaptive Home Health is seeking Physical Therapist Assistants to provide skilled PTA visits in patients’ homes under the supervising PT’s plan. You will focus on functional mobility, gait, transfers, and home safety while coordinating with the care team to optimize outcomes.

Engage patients and caregivers with practical home programs, document thoroughly in HCHB, and maintain clear communication with supervising PT to drive measurable progress and safe home-based care.

Qualificações

• Active Texas PTA license (in good standing).
• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision).
• Excellent communication and thorough documentation.
• Reliable follow-through on treatment plans and patient goals.
• Current driver’s license and daily travel across territory.

Responsabilidades

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, mobility, and home safety.
• Set/track objective progress within PTA scope and communicate updates to the supervising PT.
• Drive multidisciplinary coordination and keep care team aligned on orders.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in HCHB—defensible and efficient.
